WISE Telehealth Network: Seeks Community Health Worker (CHW)

Salary Range:  $38,833.60 – $42,770.00 annually depending on qualifications. (This is a grant funded position through September 30, 2021, with potential for renewal up to three (3) years pending additional grant funding.)

Position Summary:  The WISE (Women in the South-East) Telehealth Network is excited to announce that we are hiring a full-time Community Health Worker (CHW) for a newly funded grant from the South Carolina Center for Rural and Primary Healthcare to increase access to high value, quality healthcare in rural areas.

The purpose of the WISE Telehealth Network is to improve women’s health and well-being by providing preventive care through telehealth at local libraries in the rural lowcountry. This position requires a commitment to public health, especially in the areas of maternal and reproductive health, infectious disease, and mental health.

Please see below for more details. Position will be employed through the Charleston County Public Library (CCPL) System and must have a current driver’s license or ability to obtain transportation to travel to various branch locations in rural areas of Charleston County.

Detailed Position Description for the Study Coordinator/Research Assistant:  WISE Telehealth Network seeks a full-time Community Health Worker (CHW) who will connect community members with healthcare resources at rural library branch locations throughout Charleston County.

The purpose of the WISE Telehealth Network is to improve women’s health and well-being by providing preventive care through telehealth at local libraries in the rural lowcountry. WISE will connect women with healthcare resources in the areas of maternal and reproductive care, infectious disease, and mental health. The CHW will connect community members with healthcare resources through direct provision and referral, offering care management, and connecting women to available community and social services.

The CHW will be the point of contact for this initiative. This position will provide health education and resources to support an overall healthy lifestyle, as well as serve as a telepresenter for the direct provision of preventive care through telehealth appointments with physicians at the Medical University of South Carolina (MUSC). The CHW will help to understand each woman’s needs and opportunities to offer service connectivity with community and social services. This position will assist patients in navigating the health care system and facilitate assistance or interventions for clients between care visits. The CHW will act as an advocate for patients by staying current of community resources and developing strategic partnerships in the community to support community outreach and referrals. This position will establish peer-to-peer relationships with community partners in order to function as a link to the community. Finally, the CHW will support on-going and longitudinal evaluation research to monitor the impact and effectiveness of the program, as well as opportunities to meet women’s needs.

Responsibilities Include (but not limited to):  The CHW will be responsible for assisting the Principal Investigators in overseeing all aspects of the grant including: promoting the WISE Telehealth Network, engaging in proactive participant recruitment, assisting participants with informed consent procedures, collecting and analyzing data, and other research-related tasks. Duties include, but are not limited to: traveling to branch and mobile library locations in the rural lowcountry, providing health education, serving as a telepresenter, providing referrals for in-person services connecting women with community and social services, and meeting periodically with the Principal Investigators to optimize program implementation and deliverables. This position must be able to work independently to ensure the successful operation of the WISE Telehealth Network. This position requires a commitment to public health, especially in the areas of maternal and reproductive health, infectious disease, and mental health. Must possess the ability to empathize with the needs of the typical public health patient. The CHW will ensure the overall success of the project in collaboration with the team of investigators.

The successful candidate must be detail-oriented, participant-focused and possess excellent oral and written communication skills; must have the ability to work well with people in a group and individual settings. CHWs build trusting relationships with their clients and strive to improve the health of the patients they serve. Must have the ability to follow established procedures and to perform work requiring considerable detail and carry out assignments independently. Must be highly organized, self-motivated, and flexible to assume additional responsibilities as needed to ensure the success of the WISE Telehealth Network. Other duties as assigned. Pending funding, this position will start in January 2021.

Requirements

  • Master of Public Health (MPH), Master of Social Work (MSW), or related Master’s degree in a relevant discipline preferred.
  • Bachelor’s degree required.
  • Prior research experience preferred.
  • Must be familiar with (or willing to learn) data collection software (e.g., Qualtrics) and complete CITI research ethics training.
  • Commitment to public health education.
  • Ability to maintain strict patient confidentiality.
  • Must have excellent computer skills (e.g., Microsoft Office suite), with the ability to learn and use new technology.
  • Must use tact, courtesy and good judgement when communicating with co-workers and the public.
  • Must have stamina for periods of walking, standing, reaching, and bending.
  • Must have current driver’s license or ability to obtain transportation to travel to various branch locations in rural areas of Charleston County.
